Volunteer Personal Information Notice

Look Good Feel Better (LGFB) collects the information requested on this form, and from the references listed, to evaluate the suitability of individuals applying to volunteer for LGFB. LGFB also uses this information to communicate with volunteers about Look Good Feel Better programs, conduct training, recognize the contributions of its volunteers, and circulate its newsletters.

Volunteer Confidentiality Agreement

I agree to abide by the Look Good Feel Better Privacy Policy, as amended from time to time, and to keep confidential all personal information I may receive about program volunteers and participants, including participant interactions from workshops. I will not disclose this information to anyone, except to Look Good Feel Better employees and volunteers who require such information to administer the program. This obligation of confidentiality shall apply both during and after my tenure as a Look Good Feel Better volunteer.

Volunteer Authorization

From time to time, LGFB may wish to disclose your name, image and volunteer details in its publications or, if you work for an LGFB corporate donor, communicate to said company about your volunteer status. If this is acceptable to you, please so indicate by checking the appropriate box below. These are optional uses and disclosures of your information and image and your choice will not affect your application.

From the date indicated below until I revoke this authorization, LGFB may use and modify photographs of me as it sees fit and I grant LGFB the right, while this authorization is in effect, to the use of my name and my likeness in photographs, or in connection with advertising, publicizing and promoting the Look Good Feel Better program and other related programs in such a manner and in such media (including use on LGFB's Web site) as LGFB may choose, all without payment or compensation.

LGFB may communicate with my employer about my volunteer status with Look Good Feel Better.

I acknowledge that the information I have provided is true and accurate and I authorize Look Good Feel Better to contact the references provided by me.

I have read and understand the Volunteer Personal Information Notice and I agree to abide by the terms of the Volunteer Confidentiality Agreement.
